Every participant in Family Camp recreation activities needs to have a parental permission form on file for each child and/or a liability release for each adult.  Please note, all minor guests not accompanied by their parent or legal guardian will require the medical release portion filled out prior to arriving.

If you did not complete the online liability release and parental permission forms when you registered for family camp, you may download a fillable form and return it to us via email at famcamp@wnmdag.org.  You are welcome to print, fill out, sign, and submit upon check-in.
